This form is designed for you to submit measurements for Weddings or Group Events that have already been registered at Mannino's Tux & Tails.
In this case, the styles have already been determined and we just need your measurements and when you will want to pick up your Tuxedo to complete
your order.

Procedure: It is always best to be fitted at the Mannino's Tux & Tails Shop located in San Pedro, California.

If this is not possible:
1) Print the form below and take it to a tuxedo shop or tailor in your area for a complete fitting.

2) Enter the completed fitting information into the computer by filling in the form fields below. All fields must be complete for you to submit the form.

3) Once the form is submitted, look for a confirmation e-mail from Mannino's Tux & Tails confirming your order.

a) Your full name

b) Your address

c) Your phone number (with area code)

d) Grooms Name if a Wedding or the Name of the Group Event.

e) Date of the Wedding or Event

f) Age (if measurements are for a boy 12 and under) OR …..

g) Height and Weight (if over 13 years of age).

h) Get Directions and address for Tuxedo pick up.
